MOAA TAPS Form

Please use the below web form to notify MOAA of a member’s death. If the member was affiliated with a local chapter, please report the passing to them as well. We appreciate your help in keeping our member records up to date.

If you are the surviving spouse of the member, please list your information below. If your spouse was a MOAA Life member, their membership extends to you; providing your information ensures we can provide you with assistance and keep you informed of changes to service-earned benefits. If your spouse was not a Life member, please include in the notes whether you would like to establish a Basic Surviving Spouse Membership (and include your email address). Additional surviving spouse resources are available at MOAA.org/Survivors.

We will send an email to confirm receipt of your report, and we encourage you to contact us if there is any way we can help the family of the member. We are available at 1-800-234-6622, Monday through Friday, 9 a.m. to 4 p.m. Eastern time.

For privacy of family members, entries to the Sounding Taps column must be requested by a family member or executor of estate. Those individuals can request the listing by using this form, emailing taps@moaa.org, or calling the Member Service Center at (800)234-6622.
